You have the right to have information about you kept private. Information about you and your treatment is confidential and cannot be given to anyone except as permitted or required by law.
When you are scheduled for your first appointment, you will receive a booklet, Your Rights. This booklet spells out any rights you have as our customer. These include

To have person-centered treatment
planning
To agree with the services you are receiving
To choose or change your counselor
To complain about any problem with our services by calling our officer of recipient rights or the Michigan Department of Community Health at 1-517-335-9384
Built-in protections are established for all customers. If you are dissatisfied or disagree with services you have received, information about our grievance, dispute resolution, fair hearing or appeals process may be obtained by calling the Office of Recipient Rights, 231-689-7080, or toll-free, 1-800-968-7330.
If you are denied any of the services listed, or if you are told that you are not eligible for a service, you have the right to a second opinion. If you wish to receive a second opinion, you must put your
